This page gathers forms and handouts for families already established at Kaplan Pediatrics. Select the link you need, then view, print, or save the document. When a form asks for your child's name and date of birth, complete it before your visit or appointment.

To schedule a well visit, sick appointment, or nurse visit, call the office at (972) 943-9151 during weekday hours, 8:00 am to 5:00 pm. The office is closed for lunch from 12:30 to 1:30 pm. Saturday morning hours may be available during peak seasons.

• Behavior Visits: paperwork and visit information related to behavioral health appointments, including ADHD and related concerns discussed with our physicians.
• Well Visits: forms and guidance tied to routine checkups and preventive care visits from infancy through age 21.
• Release of Medical Records: authorization to send your child's records to another provider, school, or program that requests them.
• Health Form / Shot Record Request Form: request school, camp, sports, or daycare forms that require an updated health summary or immunization record.
• After-hours Tip Sheet: guidance on common childhood illnesses and when to seek care outside regular office hours. Read this sheet before calling after hours when possible.
• Motrin / Tylenol Dosing Charts: weight-based dosing reference for fever or pain. Confirm any dose with our staff if you are unsure.
• Flu Vaccine Form: consent and information related to seasonal influenza vaccination when offered in the office.

Allow time for record and form requests to be processed. If your child needs a same-day sick visit, call the office rather than submitting a form online. Insurance and billing questions are covered on our Billing/Insurance page.

For life-threatening emergencies, call 911 or go to the nearest emergency room. For urgent symptoms that cannot wait until the office reopens, follow the instructions on the After-hours Tip Sheet and contact the on-call service if one is listed there.

During business hours, our staff can help you decide whether your child needs a same-day sick appointment, a nurse visit, or home care with a follow-up plan.
