Welcome to Kaplan Pediatrics. We are glad you are considering our office for your child's care. Use the button below to open the packet, print or save the forms, and fill out every page for each child who will be seen. Bring the completed packet to your visit. Call our office at (972) 943-9151 to schedule your child's first appointment. Our staff can tell you which forms apply to your situation and whether we are accepting new patients for your child's age.

After you download the packet, set aside time to complete the health history, consent, and registration pages for each child. Parents or legal guardians should sign where indicated. If custody or guardianship paperwork applies to your family, bring a copy to the visit.

Plan to arrive a few minutes early so we can verify your contact information and insurance details. The office is closed for lunch from 12:30 to 1:30 pm on weekdays. Saturday morning hours are available during peak seasons. You can confirm current hours on our Contact us page or by phone.

• Completed new patient forms from the packet above
• Photo ID for the parent or guardian bringing the child
• Insurance card and any referral paperwork your plan requires
• Immunization records or a list of vaccines your child has received
• Name and phone number of your previous pediatrician if you are transferring care
• A list of current medications, vitamins, or supplements your child takes
